We're seeking a Director of Nursing to lead our clinical team and ensure the highest standards of patient care. In this key leadership role, you will supervise field nurses, provide clinical education, and oversee the full patient care cycle from assessment to discharge. Key Responsibilities: Clinical Leadership & Supervision: Guide and mentor a team of nurses and other clinical staff, ensuring excellence in patient care and adherence to best practices. You will be responsible for providing in-services, ongoing education, and clinical direction.
Comprehensive Patient Management: Conduct initial and ongoing patient assessments, including OASIS documentation, to develop and revise individualized care plans. You will manage the full scope of patient needs, including physical, psychosocial, and environmental factors.
Quality Assurance & Compliance: Ensure all clinical activities comply with third-party payer requirements and agency policies. You will participate in the agency’s Quality Assessment and Performance Improvement (QAPI) program and maintain accurate and timely clinical documentation.
Interdisciplinary Collaboration: Act as a central point of communication, coordinating care with physicians, patients, caregivers, and other healthcare professionals to ensure a seamless and integrated plan of care.
Patient Education & Support: Educate patients, caregivers, and families on managing nursing and related needs, and provide essential support throughout the care journey.
Operational Support: Handle on-call responsibilities and assist with scheduling, billing, and other administrative tasks as needed.
Requirements: A passion for delivering exceptional patient care in a home health setting.
Demonstrated experience in a supervisory or leadership role within a clinical environment.
Strong knowledge of home health regulations, including OASIS.
Excellent communication and organizational skills.

Qualifications
Qualifications
Education: Graduate of an accredited nursing program (diploma, associate, or bachelor's).
Licensure: Must hold a valid Registered Nurse (RN) license in the state or a multi-state license from a Nurse Licensure Compact (NLC) member state.
Experience: At least one year of experience as a registered nurse in a clinical setting. Prior home health experience is required.
Certifications: Current CPR certification is required.
Skills: Strong nursing skills based on accepted standards of practice and excellent interpersonal skills.
Transportation: Must have reliable transportation and current auto liability insurance.
