Smart Rehab is seeking an Occupational Therapist to provide one-on-one outpatient therapy in the homes of older adults in South St. Louis County. This is a 1099 contractor position, perfect for someone looking to continue using their clinical skills on a flexible schedule that works for them. We are a Medicare Part B certified private practice, not a home health agency, so our patients do not need to be homebound. Our therapists focus on quality care without productivity quotas. You set your caseload, visit frequency, and preferred geography. Each visit typically lasts 60 minutes and is billed per Medicare unit (15-minute increments).

Responsibilities: Provide in-home outpatient therapy to Medicare-eligible older adults Conduct evaluations, develop treatment plans, and perform follow-up visits Perform home safety assessments and provide recommendations Complete documentation in a timely, compliant manner Collaborate with families and caregivers as needed

Requirements: Licensed Occupational Therapist in the state of Missouri Strong interest in geriatrics and home-based care Reliable transportation and comfort working independently Excellent communication and documentation skills

Compensation: Paid per unit billed (15-minute increments) Most sessions generate 4+ units Initial evaluations billed separately
